Do I need to be referred
to Retina Specialists of Carolina, P.A.?
Most of
our patients are referred from their primary eye care provider for a
thorough examination by one of our retina specialists. However, we
do not require a referral for a retinal evaluation.
What should I bring to
my first appointment?
Please
download our
patient forms, complete them as accurately as possible and bring
them with you. Please also bring: Any medication that your are taking (including eye drops)If you have any further questions, please don't hesitate to call our offices for assistance. |
